Social Care Network
We help connect individuals to housing, nutrition, transportation, employment support, and community resources for healthier, more stable lives.
At REGO Park Counseling, we understand that health is shaped not only by clinical care, but also by a person's environment, stability, and access to essential resources. Through New York State's Social Care Network (SCN) program, we help Medicaid members address their health-related social needs.
All Medicaid members are screened for unmet needs such as food insecurity, housing instability, lack of transportation, utility needs, and safety concerns.
A Social Care Navigator works directly with members to identify unmet needs, understand eligibility, connect to resources, and provide follow-up support.

▪ Nutrition counseling
▪ Education and planning
▪ Pantry restocking
▪ Cooking supplies
▪ Medically tailored meals or food prescriptions
▪ Public transit passes
▪ Scheduled rides
▪ Private ride options
▪ Care management services
▪ Employment support
▪ Utility assistance
▪ Local nonprofits offering financial, legal, parenting, or social support
Eligibility is determined through screening and may include Medicaid members who:
▪ Enrolled in Medicaid managed care plans
▪ Have identified unmet social care needs
▪ May benefit from enhanced, short-term, or long-term supports
As an SCN participating provider, Rego Park Counseling helps by:
▪ Conducting or facilitating HRSN screenings
▪ Referring members to Social Care Navigators
▪ Supporting members' clinical and social needs holistically
▪ Ensuring behavioral health and social care providers work together
▪ Providing access to nutrition, housing, and transportation services
Our goal is to ensure every person receives the full spectrum of support needed for health, stability, and meaningful recovery.
