CORE Services are person-centered, mobile, and recovery-oriented supports that can be provided in the home, community, or clinic. They help individuals develop skills that increase independence, improve daily functioning, and support long-term recovery from mental health and/or substance use challenges.
▪ Goal-directed counseling and therapeutic interventions.
▪ Skill development: emotional regulation, problem-solving, managing symptoms
▪ Support navigating treatment, benefits, and community systems
▪ Daily living and self-care skill building
▪ Money management and budgeting skills
▪ Social skill development and community integration
▪ Support with employment or education goals
▪ Increasing independence at home or in the community
▪ Education about mental health, substance use, and recovery
▪ Skills training for families and caregivers
▪ Emotional support and coaching
▪ Tools to improve communication and reduce conflict
▪ Peer mentoring and wellness coaching
▪ Support from individuals with lived recovery experience
▪ Self-advocacy and confidence building
▪ Assistance with community connection and coping strategies
- Adults age 21 or older.
- Enrolled in a HARP, HIV-SNP, or Medicaid Advantage Plus (MAP) plan.
- Must meet NYS Behavioral Health High-Risk Criteria.
- Services must be recommended by a Licensed Practitioner of the Healing Arts (LPHA).
- In-Home.
